As parent/guardian, I understand that the Adventurer Club is active and includes many opportunities for service, adventure, fun, and learning. I will support the program by:

1. Encouraging my Adventurer to take an active part in all club meetings and functions.
2. Attending events in support of my Adventurer.
3. Assisting club leaders by serving as a helper when needed.
4. Not holding any individual club staff member liable in the event of an accidental injury.
5. Giving my permission for the above named Adventurer to attend Adventurer activities.

I (we) the undersigned parent, parents, or legal guardian of the above named child, understand that I am expected to be at ALL Adventure meetings and events. However, if I am unable to be located and emergency treatment is needed, I (we) give permission for adult leaders or volunteers to administer emergency treatment, contact emergency personnel, and act in my stead in approving necessary medical care until I can reasonably be contacted. I understand that should any medical bills be incurred, our family's insurance(s) will be primary and the Oregon Conference general liability insurance will be secondary, up to a maximum of $5,000.00 for one year from the injury date.

I further, on behalf of myself, my spouse, next of kin, executors, heirs, assigns, or anyone else who might claim or sue on my or my child's behalf, fully release and agree to hold harmless the Oregon Conference of Seventh-day Adventists its affiliated entities, and any of its agents, employees, and/or volunteers from any and all liability, including but not limited to any claims, losses, or liabilities due to death, personal injury, disability, property damage, medical expenses, and/or theft, that may arise from or relate to my child's participation in the Adventure Club and any and all activities.

The club takes photographs and short videos at meetings, outings, services and conference events. They are used to celebrate the children and to tell other families about the club. The club will not publish a child's last name, school, or home address with a photograph. Please check one:

Adventurer-age children remain under parental supervision at all club meetings and events (North American Division policy). This form names who the club calls if a parent cannot be reached, and the adults who may bring or collect the child on the rare occasion a parent cannot.

Club meets Tuesdays at 4:00 PM. With Hood View Adventist School's authorization, a club leader may pick up students at the school at 3:30 PM and bring them to the meeting. Check one:

The adults named above may accompany or collect my child from club meetings and events. Club staff may ask them for photo identification. I will notify the director in writing of any change.

To: Hood View Adventist School, attention Georgi Schnibbe. On Adventurer Club meeting days (Tuesdays), a Hood View Adventurer Club leader will pick up the student named below from the school at 3:30 PM and bring them to the club meeting at Hood View Seventh-day Adventist Church, 26775 SE Kelso Road, Boring. The parent or guardian will be at the meeting, or has named adults on the club's Pickup Authorization, and takes the child home from the church. When you send this form it goes to the school and to the club director.

Every Adventurer Club meeting day of the 2026–2027 club year. If a date is cancelled the club tells the school and the family; no pickup happens on a day the club does not meet.

Only an adult named on the Hood View Adventurer Club Approved Volunteer Staff List that the club director provides to the school office, not any other adult. Every adult on that list has completed the Oregon Conference background screening and child-protection training. The director gives the school the list before the first pickup and sends an updated copy whenever it changes; the school may refuse release to anyone not on the current list. The leader will show photo ID at the school on request.

I am the parent or legal guardian of the student named above. I give Hood View Adventist School permission to release my child at 3:30 PM on each date listed above only to an adult named on the Hood View Adventurer Club Approved Volunteer Staff List on file with the school, and I give the club permission to transport my child from the school to the church for the meeting. I will tell the school office and the club director in writing if I withdraw this consent or if my child will not attend on a given day.

1. In the event of an Adventurer family camping experience, a parent or legal guardian MUST accompany their own child(ren) at all times. No exceptions.

2. Parents or legal guardians are not permitted to grant permission to Adventurer staff to take their minor children overnight camping without a parent or legal guardian under any circumstances. No exceptions.

3. Adventurer staff are not permitted to accept permission or release forms from a parent or legal guardian to take their minor child(ren) camping overnight without the parent or legal guardian present on the camping trip. No exceptions.

4. Adventurer staff are prohibited from sharing a tent or cabin with children who are not their own. No exceptions.

5. Adventurers are not permitted to sleep in tents or cabins unsupervised. Neither are they permitted to share a tent or cabin with another child unless they are siblings and the parents/guardians are present. No exceptions.
